3、投標人資格要求
（1）投標人必須是來自中華人民共和國或是與中華人民共和國有正常貿易往來的國家或地區（以下簡稱“合格來源國/地區”）的法人或其他組織，在法律上和財務上獨立、合法運作并獨立于招標人和招標機構，具有相關經營范圍，有資格和能力提供本采購項目的貨物及服務的制造商或代理商（或經銷商）,總公司或者分公司只允許一家投標，不允許同時參與本項目投標，以分公司名義參與投標的，須提供總公司或具有獨立法人的上一級公司出具的愿為其參與本項目投標的行為以及履約等行為承擔民事責任的加蓋總公司公章的授權函，以及提供總、分公司的營業執照掃描件。須提供營業執照或企業注冊證明復印件。
（2）參與國際招標的投標人必須在中國國際招標網www.chinabidding.com 注冊并進入該網的供應商庫中（注冊網址：“中國國際招標網（www.chinabidding.com）”，注冊方法：請登錄網站查詢或撥打網站服務咨詢電話：010-58851111-8762（客戶服務部）、13261902952；傳真：010-59713266；聯系人：劉杰萬），境內投標人同時需具有深圳市政府采購注冊供應商資格（供應商注冊網址：http：//www.szzfcg.cn，投標文件中無需提供證明材料。采購代理機構將在開標當天從供應商資料庫里查詢本項目所有投標供應商的注冊狀態，投標供應商注冊狀態不是“有效”的，將按資格不符合招標文件要求，作資格審查不通過處理）。
（3）投標人在中華人民共和國境內注冊時，若投標人為所投產品的生產企業，必須提供《醫療器械生產企業許可證》，且生產范圍包含該產品；若投標人為所投產品的代理商或授權供應商，必須提供《醫療器械經營企業許可（備案）證》，且經營范圍包含該產品；
（4）投標人必須提供所投產品的《醫療器械注冊（備案）證》的復印件，原件備查（開標時，該證應在有效期內；若不在有效期內，則需提供該證和所投產品在該證有效期內生產的藥監局出具的證明文件）；
（5）若投標人所投產品為進口設備，則必須提供所投產品制造商出具的銷售授權書，且同時提供由產品制造商或授權的中國總代理簽署的合法有效的保修、維修承諾；
（6）投標人須自行承諾，參與本項目投標前三年內（即從本項目招標公告發布之日起往前推算36個月），在經營活動中沒有重大違法記錄，以及參與本項目政府采購活動時不存在被有關部門禁止參與政府采購活動且在有效期內的情況；與其他投標供應商不存在單位負責人為同一人或者存在直接控股、管理關系；未對本次采購項目提供整體設計、規范編制或者項目管理、監理、檢測等服務。（由供應商在《承諾函》（格式自定）中作出聲明）；
（7）參與政府采購項目投標的供應商未被列入失信被執行人、重大稅收違法案件當事人名單、政府采購嚴重違法失信行為記錄名單（由供應商在《政府采購投標及履約承諾函》中作出聲明）。
