3、投標人資格要求
投標人應具備的資格或業績：“投標人”系指響應采購文件要求、參加本次招標采購的供應商。 （1）投標人必須是來自中華人民共和國或是與中華人民共和國有正常貿易往來的國家或地區（以下簡稱“合格來源國/地區”）的法人或其他組織，在法律上和財務上獨立、合法運作并獨立于招標人和招標機構，具有相關經營范圍，有資格和能力提供本采購項目的貨物及服務的制造商或代理商（或經銷商）。須提供營業執照或企業注冊證明復印件； （2）參與國際招標的投標人必須在中國國際招標網www.chinabidding.com 注冊并進入該網的供應商庫中，境內投標人同時需具有深圳市政府采購注冊供應商資格（供應商注冊網址：http：//www.szzfcg.cn，投標文件中無需提供證明材料。采購代理機構將在開標當天從供應商資料庫里查詢本項目所有投標供應商的注冊狀態，投標供應商注冊狀態不是“有效”的，將按資格不符合招標文件要求，作資格審查不通過處理）； （3）若投標人為所投產品的生產企業，必須提供《醫療器械生產企業許可證》，且生產范圍包含該產品；若投標人為所投產品的代理商或授權供應商，必須提供《醫療器械經營企業許可（備案）證》，且經營范圍包含該產品； （4）投標人必須提供所投產品的《醫療器械注冊（備案）證》的復印件，原件備查（開標時，該證應在有效期內；若不在有效期內，則需提供該證和所投產品在該證有效期內生產的藥監局出具的證明文件）； （5）投標時要求提供原廠家的Data Sheet,并加蓋原廠家(或廠家國內分支機構)公章； （6）本項目不接受聯合體投標，中標后不允許轉包、分包。

3.Qualification Requirements For Bidder
